New Patients: Please complete the new patient packet and provide information requested, Listed/Described below, to include your clinical history. Bring the completed pages with you when you are scheduled (the date and time of your appointment) to see Dr. Tarbox. Please DO NOT send these forms back in the mail. Completing this information ahead of time allows us to see you in a timely manner, at your scheduled appointment time, and assists in providing the information necessary to fully address your health care needs.

Photo ID

Please make sure you bring a valid Photo ID.

Insurance Info

Please bring your current Insurance Card with you.

Referral / Authorization

A referral with authorization (if one is required by your insurance plan)


Please bring your Copayment / Coinsurance amounts $ (if required by your plan)

Medication & Imaging Studies

Bring ALL CURRENT medication bottles as well as copies of any recent (within 6 months) pertinent imaging studies (X-rays, MRI studies, etc.).

Physician Referral

The name, phone and fax number of the physician who asked you to see Dr. Tarbox (the referring provider)

Upon arrival, you will be greeted by our staff and your registration information will be confirmed. If your plan requires a copayment, we will ask for that you provide this amount while at check in. For your convenience, Dr. Tarbox accepts cash, personal check, or your credit/debit card, as forms of payment. Self (or cash) pay patients will be asked for the balance due at the conclusion of the day’s visit or at the checkout desk.

At your initial visit you may be asked to remove your shoes and socks. The examination of your feet is necessary so that Dr. Tarbox can evaluate your peripheral nervous system. Upon the conclusion of your office visit, Dr. Tarbox will escort you to our check-out area, where, if needed, staff will work with you to schedule your next appointment. During this time, we will also assist you in the scheduling of any needed tests or procedures, and answer any questions that you may have regarding your visit.

Our office will submit a claim to your insurance company on your behalf. If there is a remaining balance after the insurance company/(ies) have paid, we will send you a bill, as appropriate. Should you have any questions about a bill please contact our office.

We are committed to providing outstanding care/service to our patients. To work towards full achievement of this goal, Dr. Tarbox randomly surveys patients regarding their experience in the office. Should you be selected to complete one of these satisfaction surveys we appreciate your honest feedback. This will allow us to continue to improve, and better serve you.

Information Regarding Cancelling or Confirming your Appointment

Should you need to reschedule or cancel your appointment, please call us at least twenty-four (24) hours in advance to allow us the courtesy of filling your appointment/reserved time slot with another patient. Our number is (210) 641-1394.
